Scheduling an appointment is easy and convenient. We offer flexible scheduling options to accommodate your needs, including same-day appointments based on availability. To book a visit, simply call our office or use our online scheduling system. We recommend scheduling in advance to secure your preferred time, but if you need to be seen urgently, we will do our best to accommodate you. Contact us today to set up your appointment!

Any appointment requests made via “Contact Us” submission will be answered within 48-hours.  All weekend submissions will not be reviewed until Monday.  Please call our office if it’s urgent. For medical emergency, please dial 9-1-1.

For your convenience, when you call us, press 4 if you are having Covid 19 symptoms and want to speak to a healthcare provider. For medication refills, please press 5. If you would like to make a payment on your account, please press 7.

When you visit please remember to bring these documents:

  • A list of all your or your family members’ medications, including supplements.
  • Any medical records you have or can forward from previous providers and specialists.

We will advise you if your wait will be longer than normal. Our primary care medical team may be delayed by emergencies or complicated patient issues so please be understanding if these situations occur.

Non-Established Patients

For patients living outside of Wisconsin, payment is required in full before receiving services.

For Wisconsin residents, payment is also required prior to treatment. We will submit a claim to your insurance provider, and any overpayment will be refunded once your insurance has processed the claim.
